Fox River Foot and Ankle Centers accepts and works with many types of insurance carriers. As a courtesy to our patients, we will work with you to provide an insurance verification. By working directly with your insurance company, we can help you determine which services may or may not be covered by your plan. In order to do this, we must have accurate insurance information at the time of pre-authorization. Additionally, note that your insurance policy is a contract between you and your insurance company, and is subject to changes that may occur with your plan. All co-payments must be made at the time of service.
